I Read Every CDC Cruise Outbreak Report From the Last Three Years. Here’s What the Data Actually Shows

Every time “cruise ship norovirus outbreak” shows up in the news, the comments section says the same thing: cruises are floating petri dishes, never again.

I approached this like a problem with variables. How many outbreaks actually happen? Is the trend going up, down, or just noisy? And more importantly, what can I actually control as a passenger?

I pulled the numbers straight from the CDC Vessel Sanitation Program – the official government tracker, not a summary of a summary. Here’s what I found.


💡 Quick Answer

CDC requires an outbreak report when 3% or more of passengers or crew on a single voyage report GI symptoms to the ship’s medical staff.

Official CDC numbers: 14 outbreaks in 2023, 18 in 2024, 23 in 2025. That’s against tens of thousands of sailings out of U.S. ports every year.

Out of 55 outbreaks logged between 2023 and 2025, 84% were confirmed norovirus. That matters, because norovirus responds to specific hygiene habits – and that’s exactly why those habits actually move the needle instead of being a box to check.


Where This Data Comes From

I’m not an epidemiologist. But CDC runs a public tracker – the Vessel Sanitation Program – and every outbreak that meets the reporting threshold ends up on it.

This is one of those cases where the data sits in the open on an official .gov site, and almost nobody reads the whole thing. I did, so you don’t have to: the current outbreak list and the 2023-2025 archive.


What Actually Counts as an “Outbreak”

The threshold is 3% or more of passengers or crew reporting symptoms on one voyage.

Here’s a real example from the official reports: on Queen Mary 2 in December 2024, 346 of 2,565 passengers got sick – 13.5%. One of the larger outbreaks of the past few years.

Compare that to Celebrity Summit in May 2024: 72 of 2,264 passengers – 3.18%. Technically the same category, “outbreak,” but the actual scale is nowhere close.

The word “outbreak” in a headline can hide very different situations underneath it.


How Many Outbreaks Actually Happen

I expected a smooth trend line. What I got was growth, with a caveat I need to flag.

Straight from CDC’s own records:

  • 2023: 14 outbreaks
  • 2024: 18 outbreaks
  • 2025: 23 outbreaks

That’s a real increase. But here’s the honest part I won’t skip: the number of sailings and passengers has also grown year over year. More voyages means more chances to hit the reporting threshold, even if the per-passenger risk hasn’t changed.

I couldn’t fully separate those two factors. CDC’s site doesn’t publish an “outbreaks per million passengers” figure. If that data exists somewhere, I’d genuinely like to find it – happy to update this piece if anyone points me to it.


Norovirus Is the Main Suspect, Not the Only One

I went through all 55 outbreaks CDC logged for 2023-2025 and counted the causative agents by hand.

84% (46 of 55) were confirmed norovirus.

The other 16% is a mix: Salmonella (Radiance of the Seas, September 2024), E. coli (Silver Nova in 2024, Silver Ray in January 2025), suspected ciguatera poisoning (Sea Cloud Spirit, January 2025), and a handful where the lab never pinned down a cause.

2026 is showing a slightly different mix so far – and one ship in particular is worth its own section.

This is the official list as of CDC’s most recent update. The page updates continuously, so by the time you’re reading this, there could be more entries.

Two things jump out. First, 2 of the first 4 outbreaks this year were E. coli, not norovirus – a bigger share than the 2023-2025 average of roughly 5%. Second, and more striking: one ship, the National Geographic Sea Bird, accounts for three of the seven entries on this list.

That second point deserves its own breakdown.


Case Study: One Ship, Three Outbreaks, Seven Weeks

This is the part of the 2026 data that actually stopped me. The National Geographic Sea Bird, a small Lindblad Expeditions ship that runs Alaska routes, shows up three separate times between late May and mid-July.

May 26-31: 9 of 66 passengers and 3 of 24 crew got sick. Confirmed norovirus.

June 25-30: 19 of 68 passengers and 2 of 27 crew. Confirmed norovirus again – 27.9% of passengers on that sailing.

July 10-15: 18 of 62 passengers, no crew affected. Cause not yet confirmed, though the symptoms (vomiting, abdominal cramps) match the first two outbreaks.

According to Cruise Law News, norovirus symptoms have shown up on 6 of the ship’s last 10 voyages, including 3 of the last 4. The crew increased cleaning and disinfection between sailings each time. It happened again anyway.

Here’s the math that matters: the Sea Bird carries 62-68 passengers. CDC’s 3% threshold means 2 to 3 sick passengers is enough to trigger an official outbreak report. On a 3,000-passenger mainstream ship, the same 3% threshold needs 90-plus people sick.

That’s not an excuse for three outbreaks on one ship. It’s a reason not to compare “number of outbreaks” across ships of wildly different sizes without adjusting for it first.

What the size adjustment does NOT explain: repeatability. A single high-percentage outbreak on a small ship could be a fluke of a small sample. Three confirmed cases on the same vessel inside seven weeks, with the same symptom pattern, isn’t something I can wave away as noise. Something in the turnaround between five-day sailings doesn’t appear to be fully clearing the pathogen before the next group boards.

I don’t have a confirmed root cause, and neither does CDC yet for the third outbreak. But the pattern itself – sick, cleaned, sick again two to three weeks later – is data on its own, even without a lab-confirmed diagnosis attached to it.

What Actually Lowers Your Risk

I’m not going to pretend I found something revolutionary. CDC’s data and basic common sense point to the same short list.

✅ What helps:

  • [ ] Wash your hands with soap – actual soap, not just hand sanitizer (norovirus shrugs off alcohol-based gels)
  • [ ] Use serving utensils at the buffet, never your own
  • [ ] Avoid touching your face before washing up
  • [ ] Skip food that’s been sitting out at room temperature
  • [ ] Report symptoms to the ship’s medical center right away instead of toughing it out

That last one sounds minor, but it’s the single biggest factor in whether your stomach bug stays personal or becomes a ship-wide outbreak. The onboard protocols only work if passengers actually use them.

If you’re booked on a specific small ship in the near term, it’s also worth a quick check of that ship’s page on the CDC tracker before you sail, rather than relying on industry-wide averages.


When This Data Does NOT Help You

Be realistic about what this article can and can’t tell you.

  • It’s not a guarantee your specific sailing will be outbreak-free – it can happen on any ship
  • CDC only tracks voyages under its jurisdiction (touching U.S. ports) – other countries run their own systems, or none at all
  • These numbers don’t capture mild cases that never made it to the ship’s medical log
  • The Sea Bird’s third outbreak still doesn’t have a confirmed cause as of this writing

If zero risk is the bar you’re looking for, that option doesn’t exist on any large ship. Same as a plane, a hotel, or any other place with a lot of people in a small space.


What I Took Away From This

I didn’t come out of this more scared of cruising than I went in. But I did get more serious about handwashing – not because I read a tip somewhere, but because I saw the numbers pointing to exactly where transmission happens.

55 outbreaks across three years, against tens of thousands of sailings, isn’t “everyone gets sick constantly.” It’s “it happens sometimes, and here’s the specific window where it does.” And the Sea Bird case is a reminder that industry-wide averages can hide a specific, repeating problem on one specific ship.

My approach to this is the same as any other variable in travel planning: don’t ignore it, don’t overweight it either. And if you’re looking at a specific small ship, check its individual record, not just the industry number.
