Three bottles. Same shelf at CVS. All labeled “pain relief.” All priced within a dollar of each other.
Most people grab whatever’s closest or whatever their parents used. That’s not a terrible strategy – all three work. But they work differently, they have different risk profiles, and for certain situations one is meaningfully better than the others.
I went through the pharmacology on this. Here’s the practical version.
Quick Answer
For headache: ibuprofen or acetaminophen – both work, ibuprofen slightly better for tension headaches with an inflammatory component. For fever: ibuprofen and acetaminophen are equally effective; ibuprofen lasts longer per dose. For dental or muscle pain: ibuprofen wins because it addresses inflammation. For general pain when you have a sensitive stomach: acetaminophen. Aspirin is effective but mostly replaced by the other two for everyday use – its main modern role is cardiovascular prevention at low doses, not pain relief. Never give aspirin to children under 12.
The Three Players: What They Actually Are
Ibuprofen – Advil, Motrin
Drug class: NSAID – non-steroidal anti-inflammatory drug
Mechanism: blocks COX-1 and COX-2 enzymes, which reduces the production of prostaglandins – the chemical messengers that cause pain, inflammation, and fever simultaneously. Because it targets the source of inflammation, ibuprofen doesn’t just mask pain – it reduces the process causing it.
Standard OTC dose: 200-400mg every 4-6 hours. Maximum 1200mg per day for OTC use without a doctor’s direction (prescription use goes higher under supervision).
Onset: 20-30 minutes. Duration: 4-6 hours.
Key advantage: addresses inflammation. Any pain with a swelling, redness, or heat component – dental pain, muscle strain, arthritis flare, sunburn pain – responds better to ibuprofen than to acetaminophen.
Key risk: stomach irritation. Always take ibuprofen with food or milk. NSAIDs reduce the protective mucus lining of the stomach. Short-term use at OTC doses is generally fine for healthy adults. People with a history of ulcers, gastritis, or kidney problems should talk to a doctor before using NSAIDs regularly.
Also avoid or use cautiously if: you’re in the third trimester of pregnancy, you have cardiovascular disease, or you’re taking blood thinners.
Acetaminophen – Tylenol
Drug class: analgesic and antipyretic (pain reliever and fever reducer) – not an NSAID, not an anti-inflammatory
Mechanism: the exact mechanism isn’t fully understood, but acetaminophen appears to act centrally in the brain and spinal cord to raise the pain threshold and reset the body’s temperature regulation. It does not reduce inflammation at the tissue level.
Standard OTC dose: 325-1000mg every 4-6 hours. Maximum 3000-4000mg per day – but 3000mg is the safer daily limit to stay well below hepatotoxicity risk.
Onset: 30-45 minutes. Duration: 4-6 hours.
Key advantage: gentler on the stomach than ibuprofen. Better choice when ibuprofen is contraindicated – stomach sensitivity, kidney concerns, cardiovascular history.
Key risk: liver damage in overdose. This is the most important safety fact about acetaminophen and it’s chronically underappreciated. The margin between a therapeutic dose and a harmful one is narrower than most people assume.
The hidden risk: acetaminophen is in dozens of combination products – Nyquil, DayQuil, Theraflu, many prescription painkillers, some sleep aids. If you’re taking any combination cold or flu product, check whether it already contains acetaminophen before adding Tylenol. Double-dosing is the most common cause of accidental acetaminophen overdose.
Alcohol interaction: drinking while taking acetaminophen increases liver risk. The combination isn’t always dangerous at moderate amounts, but if you’re drinking on a cruise, be conservative with acetaminophen doses.
Aspirin – Bayer, generic
Drug class: NSAID (same class as ibuprofen, different compound)
Mechanism: irreversibly blocks COX-1 and COX-2 enzymes. Unlike ibuprofen which blocks reversibly, aspirin’s effect on platelets is permanent – those platelets can’t aggregate for the rest of their 7-10 day lifespan. This is why low-dose aspirin (81mg) is used for cardiovascular prevention – it reduces clotting.
Standard pain relief dose: 325-650mg every 4-6 hours.
Onset: 30-60 minutes. Duration: 4-6 hours.
Why it’s mostly been replaced for everyday pain: similar stomach irritation risk as ibuprofen, more drug interactions, and the antiplatelet effect creates bleeding concerns. Ibuprofen and acetaminophen cover the same use cases with cleaner safety profiles for most people.
Where aspirin still belongs:
If you take daily low-dose aspirin (81mg) for cardiovascular prevention on doctor’s orders – do not skip it during travel. This is a medically prescribed regimen, not an optional pain reliever.
Reye’s syndrome warning: aspirin must not be given to children or teenagers under 18 with viral illnesses – it’s associated with a rare but serious condition called Reye’s syndrome affecting the brain and liver. This warning is absolute.

The Combination Approach
For moderate-to-severe pain that isn’t responding well to one drug alone, alternating ibuprofen and acetaminophen on a staggered schedule keeps pain coverage more consistent throughout the day.
Example schedule:
- 8:00 AM – Ibuprofen 400mg
- 11:00 AM – Acetaminophen 500mg
- 2:00 PM – Ibuprofen 400mg
- 5:00 PM – Acetaminophen 500mg
This approach is used in post-dental and post-surgical settings because it provides near-continuous coverage without exceeding the daily limit of either drug.
Ground rules for the combination:
- Stay within the maximum daily dose for each drug independently
- Take ibuprofen with food
- Don’t add any combination products (Nyquil, etc.) that already contain acetaminophen
- This is for short-term use – not a long-term pain management strategy
Fever Specifically: When to Treat and When to Let It Run
A fever is your immune system working. Not every fever needs to be reduced.
Low-grade fever (99-100.4°F / 37.2-38°C): generally doesn’t require medication unless it’s causing significant discomfort. Staying hydrated and resting is the first response.
Moderate fever (100.4-103°F / 38-39.4°C): ibuprofen or acetaminophen are appropriate. Both bring fever down by 1-2 degrees within an hour.
High fever (above 103°F / 39.4°C): treat with medication and monitor closely. If it doesn’t respond to standard OTC doses within a few hours – medical evaluation.
Fever with other concerning symptoms – stiff neck, severe headache, confusion, rash, chest pain, difficulty breathing – is a medical situation regardless of the temperature number. On a cruise ship: medical center. Don’t wait.
For children: fever thresholds and treatment approaches differ by age. The general pediatric guidance is that fever in children under 3 months requires immediate medical attention. For older children, Tylenol and children’s Motrin are the standard – aspirin never.
Headaches Specifically: Type Matters
Not all headaches respond equally to OTC pain relievers.
Tension headache – the most common type. Pressure or tightness around the head, often from stress, dehydration, or poor sleep. Ibuprofen is typically most effective. Hydration is equally important – many tension headaches on a cruise are dehydration headaches from sun exposure, alcohol, or simply not drinking enough water.
Migraine – pulsating, often one-sided, sometimes with nausea or light sensitivity. OTC options work for mild migraines. Ibuprofen 400-600mg or acetaminophen 1000mg at onset. Excedrin Migraine combines acetaminophen, aspirin, and caffeine – the combination is specifically formulated for migraines and works better for many people than any single ingredient alone. If you’re a migraine sufferer, this belongs in your travel kit.
Cluster headache – severe, one-sided, around the eye. Very painful. OTC medications are usually insufficient. If you know you get cluster headaches, talk to your doctor before travel about prescription options.
Caffeine withdrawal headache – common when travel disrupts your coffee routine. The fix is caffeine, not ibuprofen. A coffee or a caffeine tablet resolves it faster than pain medication.
The Dehydration Factor
Worth its own mention because it’s the most common and most easily prevented cause of travel headaches.
A cruise ship in Mexico is hot. You’re likely drinking more alcohol than usual. The pool deck, port days, walking around – you’re losing more fluid than you realize.
Before reaching for ibuprofen for a headache – drink 16oz of water and wait 20 minutes. A significant percentage of headaches on cruises are pure dehydration. The water costs nothing and has no side effects.
If it’s still there after hydrating – then medicate.
What to Pack
- Ibuprofen 200mg tablets (Advil or generic) – primary choice for most pain and fever situations
- Acetaminophen 500mg tablets (Tylenol or generic) – alternative when ibuprofen isn’t suitable, and for the staggered combination approach
- Excedrin Migraine – if you’re prone to migraines or want a stronger headache option
- Aspirin 81mg – only if you’re on a daily cardiovascular regimen prescribed by your doctor
Generic store-brand versions of all of these contain identical active ingredients to the name brands. The active ingredient is what matters, not the label.
